A & T Healthcare assists the referral source in case management pre-discharge. This service provides a continuum of care from hospital to home.

Placement Procedures: Once a referral source places a request for coverage, A & T Healthcare requires the following information:

• type of service
  

• hours of duration of service
  

• special skills needed for the patient
  

• and any other pertinent information
